WHEN TO USE THIS FORM

Community Banks should use this form to record items that you gift to communities that are outside of your grant, scholarship or sponsorship activities.

Examples include contribution to appeals, donation/lending of equipment, provision of expertise and volunteering within normal working hours.

These contributions do not follow a competitive application process, must not be provided in exchange for reciprocal benefits and do not require acquittal.

REPORTING YOUR DONATIONS

Data collected by this form will help you meet internal reporting needs and Business for Societal Impact (B4SI) assurance standards.
